STAY AWAY if you love your loved one! Some of these folks are unprofessional, ill-trained and shady. We took my 91-year old dad away from there because of very poor communication between staff and administrators including the head therapist Jonathan. Their lack of knowledge in several areas motivated me to call their headquarters where I finally was able to talk with someone who knew what they were doing. I am grateful for that but you are making a terrible decision if you send someone there to be cared for. Bad independent experiences with my mom, sister and myself validate that they need some very serious personnel changes.

My mother received excellent care from all of the nurses and therapists(especially Tee Simpson). The administration frequently checked if all her needs were being met and tried to be accommodating. The facility is very well kept and besides the Covid rules... very welcoming.

This facility (building) is beautiful and well maintained. However, the level of care administered and incompetence displayed is UNACCEPTABLE. I regret placing my loved one at this facility and wish I would have checked them out sooner than later. Please do not ignore the red flags! These people are not professionals and do not have the medical experience nor expertise to properly care for an elderly person with diverse medical needs.The level of NEGLECT is truly sickening...UNDERSTAFFED. Caution: Do not focus on how beautiful the facility appears. Please take the time to properly vet this establishment. If you do decide to place your loved one at Methodist Transitional Care Center, be sure to visit as often as you can. Trust me, whats hidden will definitely come to light. God bless you and your loved one!

This Transitional care center is beautiful, at first I thought that the staff didnt care about the residents, I found out you just have to get to know the staff. You can also bring your love one nutritional food. They have a Beauty Salon if you need to want your family member to get sexy. And, the physical therapist, and the occupational therapy are the best, they really work with the residents short term especially. I ask God to help the Methodist Transitional Care Center Staff to love as they love their family members, and to treat each human as they want to be treated.

Stay away from this place! Nothing but negligence since day one. Administration is horrible especially Jonathan the case manager. There are a few good people like Bobbi(nurse), Cathy(CNA), Tom(PT), and Atonya(nurse). The bad definitely outweighs any good. Please don’t take your loved ones here.

The therapy staff that Ive met ( Tom & Camille ) have done a fantastic job with my mom. The nursing staff along with moms doctors are for the most part always friendly and attentive! Glad we chose this one and wish we could take them with us!!!

I’m going to keep it simple.Therapist— 5 star hands down. Tom and Dereck are the BEST therapists.Food— 2 1/2 stars. Can’t blame the cook on the items they have to prepare, that it’s served for the most part cold, or dull, dull, dullCNAs— 4 star Those that care, show it. Those there for a paycheck, showed it (2 1/2 star).Doctor’s- 4 star talk to them, they’ll listen. Don’t complain first.Nurses— My experience was 4-5 star with all the nurses that cared for me. Shout out to BritneyA few glitches along the way: Turn over is constant, nights and weekends are severely understaffed, “fill in” personnel are present, decisions should be clarified with the pt/ot therapists before executed. Housekeeping needs retraining. The communication between staff and upper management needs a severe make over. The owners need to be more personally involved before the negative reviews out number the positive.I came back a second time based on my first experience but was disappointed. However, I collected my thoughts and voiced them. If you want to make a change, or at least hope you do, tell your story there. These facilities are not financed like a nursing home, in fact tremendously less, and yet they are held at higher standards. Work with them and know every place has its wonders….and woes.

I left a negative review on here several days ago and, after some frank discussions with the administration, I realized I entirely underestimated this place. As other reviews have stated, most of the experience is good, but you have to get to know the staff here and they’ll work with you and your loved ones on almost anything you need.The facility may be a few years old by now, yet it still looks and feels brand new. Most of the nurses and CNAs, as well as occupational/physical/respiratory therapy staff here, are friendly and helpful. I haven’t had a chance to work with speech therapy this time around.In my case, I’m currently on contact isolation, so I haven’t been able to leave my room to interact with the other residents and staff for the time being. Still, I was informed yesterday by an administrator that there would be some changes and updates coming soon. For those who do have to stay in their rooms, though, I do wish there were at least a few options for in-room activities and entertainment: a deck of playing cards, dominos, checkers, chess, books, puzzles, movie and popcorn days, possibly a PlayStation or Nintendo Wii. The latter could really be used for great exercise and hand/eye coordination activities.I gave my review 4 out of 5 stars because this facility (as with most skilled nursing facilities around the country) is not without its limits, be it accidental or intentional. My chief complaints about Methodist Transitional are the food and the highly inadequate staffing on nights and weekends.First up, the food. While it is (for the most part) healthy and tasteful, there is no longer a variety in and freedom of choice with menu options as there once was when I first admitted several weeks prior, at least as far as I know. The kitchen staff tries their hardest to make each meal delicious, but it was *perfect* when the dietary manager came by our rooms each week to sort out weekly menus.Staffing, on the other hand? I don’t believe for one solitary second that Methodist Transitional itself is at fault for this. I do think, however, the staffing responsibility lies squarely with the way the laws and regulations are structured in the state of Texas; perhaps some corporate meddling as well. Y’know, bureaucracy and all that nonsense. I was at another skilled nursing facility up in McKinney a few years ago, a much older place built in the 1970s, and experienced the same low-staffing and turnover issues there. The reality is there’s not that much of an incentive to work in skilled nursing facilities as there are full-blown hospitals or long-term acute care centers. Weekdays are nearly adequate for most things residents need here - pain management, bathroom visits, ice or water, and snacks. Nights and weekends, however, are another story. Methodist Transitional *can* at random be severely understaffed during nights and weekends, which often makes it difficult for hourly rounding, and to get call lights answered in a timely manner for things which residents need; say, for example, to have the linens changed or pain management.

DO NOT TAKE YOUR ELDERLY LOVE ONES HERE. Especially if they can’t move or walk on their own. My grandma was there to get therapy to learn how to walk again and then we were going to take her out. Unfortunately, she did not make it. When she arrived she had a womb vac for her incision that got infected due to a emergency hernia surgery. They did not clean her womb like they were supposed to. I regret taking her there. When I went back for her belongings they couldn’t find her Jesus crucifix which she loved and would use it for comfort. In one of the bags her clothes were growing mold on it. I have pictures but will not post yet. Then one Friday night her oxygen level was low and they were going to wait until Monday to look into the issue. We immediately told them to take her to the hospital. She was positive for Covid and was bleeding from her colon.

Beautiful, new, and clean facility but the staff is woefully inept. Had a hard time getting someone to answer the phone. When I did reach someone, I was transferred to an individual’s voicemail with no name and of course, I never received a call back. The food was horrible and served cold. My relative had to return it on several occasions. Communication between the staff was non existent. I’m still waiting on the medical equipment and Home Health Care and it’s been more than two weeks since I was discharged. The only positive experience was the therapists. They did an exceptional job! I still would not recommend this facility. They need to undergo some immediate firing, hiring, and training!

Beautiful new facility with specialized programs, a focus on transitioning to the next level, and strong clinical team! Would highly recommend to anyone looking for post-acute care and/or rehabilitation.

Unfriendly staff can make the isolation of this place so hard. She sounds so sad when I talk to her. All of the staff at Charlton were smiling and friendly. I wonder why this place is so different? The people that come in to give her physical therapy are very caring and friendly so that helps a lot. But the food is pretty bad. They gave her ham and she’s stage 4 kidneys and strict low sodium. The nurse told her she shouldn’t have eaten it when her leg swelled.

Super clean facility with a great staff of people who care about the people admitted. If you want your loved one to receive rehab, this is definitely the facility to do it.

Not the place you want to be, my Godsister was there, workers rude and slow to bring meds, and doesnt return calls if you contact the front desk...Do your homework...dont go or put your love one there.....They dont care, they said skilled workers, rehab, not so!

The nurses and administration are lazy. You can not get the administration to call you back. What are they afraid of? The nurses sit in their seats when you ring for help. They wait for the Aides to come and assist you.

WARNING!!! DO not take your loved one to this facility!!!! My sister was there for 3 days and had the worst experience with the nurses and techs! They were rude and had no compassion!! They do not answer the phones. We have made several calls to the director Brandon Ewertt and he has not returned any calls.

This Skilled Nursing and Rehabilitation in DeSoto, TX has the most dysfunctional administration and front office I have ever seen. No well-meaning nurse can overcome this ineptitude in management. We had trouble getting information from the very start. Could not even get water for our loved one who was stuck alone in the COVID quarantine ward without that being explained to us. Just nothing at this place works as it should especially communications to the patient and family. Our loved one was there a week before we could get even a brief phone call to discuss a care plan and the call was lacking in detail or even basic knowledge of the patient. Night nurses really try and are nice. Day nurses are impossible to reach and are probably not given the support they need. Nurses have had to use their own personal cell phone to communicate, and I do not think that meets HIPPA standards. The administrator should be fired. I do not recommend Methodist Transitional Care of DeSoto for nursing home care, physical therapy, long-term skilled nursing, or anything. Image is of one of the frequent failed calls to their main number.Response to 3/27/2021 reply: We’ve spoken with administration and the initial sales person. Things were left to disagree.
