My referring physician said his office would contact me to setup an appointment. They never contacted me. They sent the referral over a 2nd time and finally I had to contact them got an appointment setup over the phone gave them all my documentation. Then when the appointment came they refused to see me because I forgot to bring it even thought they already had my information. Plus this is over a half hour drive for me. Not trying again. Will find another provider closer to me. Bad practices for someone in the pain management field.
