FCV is a wonderful place to be! Its like one big family here! The staff are so kind and helpful, and the new renovations have made the facility so modern and welcoming.

First Community Village is great! They have beautiful outdoor areas, including many paths to walk and enjoy the space, and the staff is very friendly! I had a great experience visiting this tight-knit community!

I would give less than a star but I can’t unfortunately. Jennifer Morrison is a supposed licensed social worker at this facility and the most incompetent I’ve ever had the misfortune of dealing with. The head of the physical therapy department doesn’t show much promise either. This facility is releasing a resident WITHOUT PROPER AFTER CARE which is a case of negligence in itself. I expressed concern that the resident was not able to care for themselves in a home setting, as they’re not independent enough to attend outpatient services. They are proceeding with the discharge, on a weekend, regardless of the after care plan. This issue was addressed with the aforementioned “professionals” and I quote because professionals handle business when it pertains to their job and or position. The director of nursing Debbie, was made aware of the situation and failed to make the proper provisions in order to effectively and properly care for this individual after they leave the facility. Incompetence sums up the whole administration. This isn’t the only time The Chelsea and The Burkhardt have demonstrated their absolute and utter incompetence. The facility, aesthetically, looks to be pleasing. However; the services lack any hospitality. I’ve called Adult protective services, I’m calling the Ohio Attorney General, the State board of health, the National Association of Social Workers, and the American Physical Therapist’s Association. I’m not letting up, I will continue until they pay HEFTY fines and are investigated till the Ohio Dept. Of Health become regular staff in their offices. Period.

The staff from laundry to the physicians are impeccable. They are very positive, sweet, phenomenal, awesome and it shows that they enjoy their job.The facility is so clean it is like a 5 star hotel no kidding.Thank you all for you help and services.

I really thought this place would be a great home for my mother in law when she was first in the assisted living. When she started to fail, and went under hospice care, they gave us 2 weeks or less to get her out of her room after 6 years and told us she would be put in their healthcare center and get rid of her cat (as they claim....she doesnt know she has a cat anyways) Really??? This is the dignity and respect she is awarded after years and thousands of dollars spent living here. Appalled! I dont feel there was adequate communication that occurred between the aides and the nursing staff. We could rarely get a hold of a nurse, and they would call us back late at night when we had a concern. We hired a private duty company to care for our family member, and the regular staff there did not treat the outside company help with respect. They would ask for help in transfers and care supplies and the aides a lot of the time wouldnt come to assist. I am pretty sure the Burkhart is called Assisted living but once we hired private duty, they neglected to help and left it all to the other company. We really felt like they just pushed her out once she started to decline in health, and were never contacted by the nursing staff again when we decided to move her into a facility that would allow her to be with her beloved cat. I will compliment the social worker there, Gina Rogers...she was wonderful and helpful, but I cant say the same for the rest of the staff. Sad.

If I could give it a negative star I would. My mom hasnt been here 24 hours and the staff is treating her like crap!!! My Mom is not senile but is being treated that way. Cant wait til she is done with rehab so she can get out of this hell hole!!! And this is a church??? What a joke!

If I could give this hellhole negative stars it would be a -10!!! Staff does nothing to answer patients lights or buzzers even while they are standing around. I addressed this problem with the nurses and was told we just came on duty...total incompetent people. Wouldnt take my dog here to die...so sorry but true. If you want your family member to be comfortable DO NOT GO HERE!!!!

Beware when you choose this facility that the facility and the doctors that work in the facility bill separately. I was surprised when the Dr. bills started arriving that my secondary insurance (Medicare Supplement) was not a provider for the physicians group that were working & billing at the facility. When they said they accepted both my insurances, that meant the facility accepted my insurances. They never mentioned that there is a separate a physician group working within the facility. Check the physicians group on your secondary insurance because the admitting people dont mention this twist when you check in.

I wouldn’t recommend anyone going here they never have enough staff for the residents they don’t care about employees . lie to employees and residents terrible place
