Rocky Mountain Care was a wonderful place for my father to spend his final days. The staff was amazing, kind & caring. We as a family were treated with respect & concern. I am so grateful there are people in this profession. I would recommend this facility to anyone who is in the need of this service.

The staff is ornery and ignorant. The care is poor and neglectful, I had to ask the staff several times just for basic necessities on behalf of my mom.

My mom was here in the rehabilitation part of the facility and enjoyed her stay. The nurses were all really sweet and one particular CNA Auniece Bumpers was her absolute favorite and will she will miss her. I highly recommend this place to any loved ones.

DO NOT send your loved ones here! My parents both fell and needed physical therapy to get back on their feet. They stayed for 1 month and both say it was the worst month of their lives! They are extremely short staffed, the staff they do have are burnt out and give the impression they hate their jobs. The food was terrible, the staff are impossible to get ahold of, they act very put out with everything. This place was a nightmare.

This care facility is a joke. My grandma was staying there and all she did is complain about the food and staff. We would take her to the doctors appointments and they would never give us all the information on my grandmas medication and blood pressure. Whenever the doctors would call while we would be waiting in the office they would either hang up or just not answer. We ended up taking my grandma out of the facility and moving her due to this happening constantly. I would never recommend taking a love one to this place EVER

The food was terrible. The kitchen was inconsistent with special instructions. Meals were not designed for diabetics. Alot of carbs. The physical therapists were fantastic. Daytime care was wonderful but it changed after 7:00 pm. Response times took longer than usual.

I have stayed at 3 different skilled nursing facilities over the past 6 years. I have had a total of 6 stays. This facility far surpasses any that I have previously stayed at!!!The staff, the therapists, the rooms, the food, the facilities were far better than anything I had previously experienced!! I ended up staying at Rocky Mountain for 2 months. I wished I could have stayed longer!! I was so sad to leave!!!There were certain CNAs that encouraged & motivated me to just walk!! Id be so proud that I walked a half mile one day & Kyle would say, How about a mile tomorrow? OK! Not really thinking it was possible because it took everything out of me to do just 1/2 a mile that day! But I would do it!! I didnt want to let him down!!! Every night when I made my rounds, Jayden would ask what # lap I was on & would encourage me & be proud of me!The most encouraging CNA was never even my aid!! He occasionally brought my meals, but never worked with me directly. He just learned my name & said hi & encouraged me every time he saw me walking laps. Thank you Kyle for helping me build up from barely walking across the room to walking 2 miles a day!!!! I LOVE Rocky Mountain Care-Clearfield!!! 💕 I just wish I had found them 6 years ago!!!!

I was in the rehab side and am very happy and grateful to all the staff. I was treated with dignity and respect. Not once did I feel like a burden,even when I kept loosing my call button :) I felt genuinely cared for and encourged. I hope someone reads these really thank you for helping me get back on my feet, literally.

I’ve been a double amputee (almost 8 years)and incomplete quadriplegic for 18 years. Rocky Mountain by far has been the nicest place inside With great staff that actually care and listen to what you have to say (care wise) they follow my instructions and do things how I like them done.

I am not pleased with this place. We currently had my Grandma there and I will never put her back. The staff is not great, they do not give enough attention to their patients and the food is horrible! They couldnt even put an IV into my grandma, it took over seven tries and still didnt get one in. All they did was leave both of her arms all bruised, blue and black. They had a pill just lying around on her table, and no one knew who put it there let alone what kind of pill it was! They had her so high, who knows what could have happened with that one little pill. I wont even get into the fact that someone lost her blood! We had to move her back over to the hospital because of everything, which at the moment it was the bast decision we could have made. She is doing so much better now that she is not there. She is moving around and doing more therapy at the hospital than she was at the rehab. I am glad to say that we will not be taking her back.

My mother was left off of oxygen twice. She requires oxygen 24/7 for survival. One of the nurses lost one of her inhalers. I discovered this and it took two days for it to be replaced. There is not enough staff to answer patient call lights. I will say the food is better than other care centers. I did address these issues with management. They said they would work on improving but bottom line, my mother could have died without her oxygen. I felt like I had to be there every day checking on my moms welfare.

There have been a lot of changes as of lately at RMCC. But this has led them to have some of the best team they have had in a long while. My grandmother-in-law is here and thriving. The staff love her as there own and take excellent care of her. ♥️

RM will be a rehab and a short stay for my mum after knee surgery and awaiting to be a permanent resident at Chancleor Gardens. They have helped me with paper work and all questions as well as popping in and out of the facility to see how the care there is and staff around the clock care. Residents seem to love it there over other facilities and its a bright happy place. I feel at ease with my mum being taken care of at RM. Of course I will be there quite a bit and will have plenty to say if anything was to arise up. With both of us at ease I believe we will have a pleasant journey.

This is beautiful and clean inside and out. I would recommend it. I couldnt ask for better care than what I received from everyone there. The food was so good. I looked forward to each meal. It was GREAT!!!!

This place is a complete joke. My Grandma is there now (we are trying to get her moved) after having knee surgery and they are just horrible. They have forgotten her pain meds twice, they left her waiting for two hours before the would answer her call light to take her to the bathroom. I mean come on she just had SURGERY!!! She cant get around by herself. When we talked to them they just make up lies. Horrible horrible horrible place. If you have a loved one that needs care DO NOT take them here or any Rocky Mountain Care center. They are all awful and should be closed down!!!!

Our family wasn’t impressed with the level of urgency in getting our loved one comfortable on hospice. It took hours before James put “orders” in and “went to his car” for a long time while our loved suffered in a lot of pain.

There are some individuals here that are really great, but overall, I would say never send someone you love to this facility. My grandma was constantly given her meds wrong. Too late, too early, too much, missed all together, and it caused tons and tons of problems. They also keep saying they are short staffed but I’ve read reviews for the last 4-5 year and many say that they’ve been saying that for years. I don’t buy it. I feel like they are lazy, a little dishonest, and mostly want to do the bare minimum that they have to and sometimes not even that. Very very unfortunate.

My grandma was there for a few weeks after a fall. She said the food, the therapy, and the nursing were great! Whenever I came and visited her the place was very nice and clean.

This place services are terrible. The staff would rather stand around talking then help patients. Please don’t leave any elderly patients there they call or ring for help NO one comes.It can take over 30 mins to get help. If you are recovering for hip or knee replacement and Not picky on the crappie food ,old worse out beds, tv that works half the time bad plumbing. They make this place sound GREAT and upper management is the worst. Beware enter at your own RISK..

I would give negative stars if it were possible. My dads health declined while at Rocky Mountain Care. We had to have him hospitalzed. He could have gotten sepsis because of the way they handled his dialysis catheter and the food they feed the patients is terrible. Theyre severely understaffed and it is impossible for 1 nurse to care for so many patients at a time. His room was dirty and his bathroom was disgusting. It was cleaned only after we complained.

I work at Davis Hospital and have had patients brought in and basically abandoned by their Rocky Mountain Transporter. Before patients have been checked in and assured they are in the right location for their procedure, the transporters will take off and then are very difficult to get a hold of after. Then when patients need to be picked up, it takes much longer than necessary to get a hold of someone and sometimes the patients have waited hours to be picked up. I would never let a loved one stay at this center.

I did clinicals there about 3 years ago but from what I’ve heard things haven’t gotten any better. It’s true there were some CNA’s and nurses that deserve a 5 because they went above and beyond. In my case I had a 1 star nurse. I was there for 8 hours and maybe saw her for 2. When I did see her or was helping do cares she would yell at all the residents and cus at them if they didn’t do what she wanted. In the time I was trying to find my CNA I observed some of the others and saw similar. Props to those 5 star caretakers you most definitely make the lives of those residents better.

I walked in and they had my dad in a chair and he was slumped over he was sleeping in the cafeteria with no one next to him. They left him in bed with no t.v. on so he was just staring at the ceiling. They have horrible care for the patients.

As a physician, I discharged a patient back to RMC. I was called by the facility asking to cancel an order for Carb count insulin because we just dont do this very often. What was best for the patient wasnt taken into account because it is easier to just not do this. I have 13 year old type one diabetics that can do their own carb count insulin. Not impressed.

There were many times that 1* would be more appropriate. But not every moment or everyone was a HATE experience. Actually...The Spinach soup was loved by the family. There were a handful of nurses that were wonderful and we loved their services and association. However, in a 49 day period we did not have 48 hours that meds were given properly. We received many excuses. We are short staffed, I am new, We have new nurses on, Our admin is new, The med instructions are difficult to read, That medicine is not in the cabinet (but it was, my sister was called and went down at 8:00pm and located it in the narcotics part of the drawer, meds was scheduled at 7) Another resident has fallen and I was unable to get here within 45 minutes of the dosing time, Ill be back another resident has an emergency, Dr B is at Bear Lake. Ill shut your light off and the head nurse will be in to discuss.... We met with Head nurses, Nichole, CNA nurses, and other admin to solve the problems with little success.We now have authorities from Davis County that is working with and auditing the nursing staff, and other inappropriate actions of the facility.I appreciate the thoughtfulness of the nurses that have been kind and gentle with mom. For every 1 that has made excuse or been unable to locate or care for mom there has been 1 that has been very efficient and would get a 5 star. State law allows for a 5% error. I think we are at a 50% error. There license is in jeopardy.We are working with them and the Davis County authorities to rectify the concerns. We hope we can improve the services. If we were to do this again...RM would not be a place of choice. Why dont we move to another facility? Because we have been told these are common errors in skilled facilities. Fortunately, mom has a good mind and knows what meds she should be getting. Many of the residence dont realize how incorrect their meds are. Moms Dr at the UofU was furious and wrote a letter to the facility about his concerns.We are working to make RM a place of choice for others in the future. Thank you to those who have met and are moving forward to correct these concerns.

They dont have enough doctors or nurses for the size facility. I was not impressed with the care my father received when he or I requested it for him. There were some employees that obviously cared about the patients under their care and others that were too busy to care.
