I was very pleased with the center. The staff were friendly, helpful & professional. The nursing staff were caring & always kept us well informed. The Occupational Therapist, Sheryl was amazing. She always kept us up to date on the evaluations that were done for our loved one. I would highly recommend this facility.

I am grateful for the care my Dad is receiving here. Each step of the way has been a new experience for me and the administration and nurses have been patient and have helped me navigate the process. While my Dad would clearly prefer to be living independently and even in his limited capacity, he says that the team is nice to him and he is being taken care of. They have even reached out to me proactively on their personal cell phones so we can speak.

Very disappointed in this place!! My Mom was not even here for 24 hours. She was here for rehab but the living situation was not good! We were told she was offered a private room after we told them we were taking her out but that never happened! The toilet stayed stopped up and had to be shared by 4 people! The facility is rundown and needs some repair and fixing up . The outside is not inviting! Not what we expected at all!! Would not recommend!!

My mom has been a resident at Brian Center for many years now, during that time we have all become friends and like an extended family. Especially through the Covid-19 pandemic. Thankfully covid has stayed out of the building due to the diligence and safety measures of all members of staff.Thank you to all the Brian Center staff for your care and compassion.

I would not go here at all costs. Speaking as a nurse of 43 years, the care was appalling and my sister-in-law checked herself out after staying 29 hrs. The care is also dangerous, risk for pulmonary embolism post op due to inability to get out of bed . My family member arrived at 5 pm to the rehab unit on a Friday, 2 days after surgery.. She requested pain med at ~ 9:30p and the nurse said meds would arrive by midnight. I also called to verify this at 10pm and spoke to Yolanda. At 9am the next day she had to call her spinal surgeon and was then given a pain med which had been ordered every 4 hours. She had a major spinal fusion surgery. She left hospital ambulatory with use of walker. Upon arrival she was told she had to use a bed pan until 9 am the next day when she would have PT eval. Guess what...no PT eval and still no walker, no ambulating unless in a w/c which meant twisting and turning the surgeon told her not to do. She was told eval would be Monday. She needed to be ambulatory post op. Total incompetence on so many levels.

I would like to give this place a High 5 the entire staff from house keeping and food staff and extremely shout out to the entire cna and nursing staff on all shifts and everyone that is above they are doing absolutely the best keeping us Covid free I would highly recommend this facility to anyone THANK YOU BRIAN CENTER FOR YOUR GREAT JOB YOU DO TO KEEP US SAFE AND READY TO GO HOME

I arrived at the Brian center in June 2019 due to a fall and injury to my leg. All of the staff here are very good at what they do. The therapist are the most caring and wonderful people to work with, and the rest of the staff that helped to take care of me are some of the most kind and caring people I have ever been around. I have had to extend my stay due to my circumstances and I feel truly blessed to have found a place that is taking such good care of me. The food 🍱 is good. There are activities that are fun and interesting. I would definitely recommend considering this facility for anyone needing care or assistance.I will be celebrating my 59th birthday tomorrow 3/31/21 here at the Brian center, so I just wanted to update my review regarding my experience here since I first arrived. All the staff continues to show a level of care and concern for my well being that just continues to warm my heart each and every day. The attention to protocol that was maintained during the whole pandemic period has just been a true wonder to see. The dedication that the staff has shown through a period of time like no other leaves nothing to be desired. The therapy department has continued to check in to make sure that my quality of life is the best that it can be, and they have provided such great assistance throughout every day whenever I require anything with a therapy related issue or just with my normal day to day activities. The kitchen staff is very accommodating when there are foods that I don’t want, and they continue to do a wonderful job serving me with my preferences as we have set up. My room is cleaned every day too. From top to bottom I am very thankful for everyone that helps me here at the Brian center, and again as I did a while back... would recommend to anyone with the need for assistance to check out this place as an option.

I can’t say enough great things about the staff at the Brian Center in Clayton. Regardless of the Medicare rating this facility in my mind and my heart is a 5 star. The PT OT speech therapist and wound nurse are the best. These people show compassion and are all about healing and caring for the residents. The office staff is equally caring about not only the residents but the families of their residents. I would highly recommend this facility to anyone and everyone. They took such awesome care of my mom. Thank you ALL from the bottom of my heart!

Brian Center Health & Retirement/Clayton should be investigated. An absolutely terrible place. If your loved ones cant speak up for themselves DO NOT entrust their staff to take care of them.

My 90 year old mother in law has been in Brian Center for two and a half years. We toured other facilities in our area and Brian Center was certainly the best! We are very involved in her care and are there often to visit. The staff is caring, compassionate, attentive and engaging. They are professional and keep us well informed at all times. It is clean and they serve the residents well balanced nutritional meals. If a meal is not to the residents liking.... there are several alternatives available at any time.They celebrate all of the holidays and the residents birthdays. They have daily activities including, but not limited to coloring, memory games, bingo (with prizes), and ceramics.They also have laundry service available to the residents, which my mother in law loves to send her laundry out and get it back the next day. Phone and cable TV available to each resident.I dont think there is such a thing as a PERFECT convalescent center because its not the same as home. But the Brian Center of Claytons staff sure does the best they can to get as close to perfect as possible.

My 86 year old Dad recuperated at the Brian Center several times for cancer and heart related issues. The staff are very caring compassionate and professional. Theres no better nursing home in our area. That said, family members must stay on top of your loved ones care. Stay involved and you will find people here going the extra mile to be the best at what they do.

The pros: the physical therapy team is great and the food is good.The cons: very old and in need of renovations, understaffed and lack of compassion in many of the staff. Call bells consistently ring for 20 minutes or more before anyone checks on the resident. Baths are not given as they are supposed to be. My grandmother was there a week and got 2 sponge baths. She was supposed to get one every day and a shower twice in that week. Be sure you go visit before you place your loved one here.

This is the second stay for my mom in Rehabilitation at the Brian Center. They have by far, the best rehab team. The physical therapist are well trained, and they go above and beyond their duties. The nursing staff, Valerie, have been outstanding to my mom, as well as the aides. The Administrator, Kevin, is available to answer any questions at any time and believe me, I ask. Im there basically every day. Also, the Admissions, Billing and other staff members have help me tremendously.

Physical therepy team was awesome with rehab. Michael the social worker was terrible for follow up and communication. Care for my father was terrible. He declined in every aspect. Several falls, scrapes and mental issues do to changes in medicines without consulting us and knowing his history. I would not recommend this facility period

My grandmother was a resident at the Brian Center of Clayton & let me just say if i EVER i have to place another family member or recommend a facility to someone it would be the the Brian Center! My Grandmother had become very sick and weak and ended up breaking her leg after a fall and needed therapy to relearn to walk Ms. Mary in the admissions department visited us in the Hospital and i knew right then where granny would be going upon arrival Ms. Mary met us at the front entrance she made us feel like we were at home! she stayed with us while we got granny all settled in and made sure that we all felt right at home she went above and beyond for us.also want to say i can not thank the amazing therapy team for the hard work that they put into teaching my grandmother how to walk again her surgeon said with her age and health issues the chances of her walking again were less than 50% and they worked so hard with her day in and day out that my granny WALKED out of the Brian Center on her day of discharge!! I honestly can not say enough good things about the staff we only had a couple issues with one of the nurses aides attitude but when i addressed my concerns to the Administrator Ms. Janet she was on it and resolved everything very quickly! She even called me a couple days later to check and make sure that i was not having any more issues. The process of my grandmother discharging was so smooth and clear her case worker Ms. Sue & Ms. Chelsea made sure that granny would continue to receive therapy at home & made sure she had everything she needed any equipment we didnt have, a nurse to come out and check on her a couple times a week and even scheduled her up with a follow up appointment with her Dr. for a couple weeks after she she left they took ALOT of stress off of us so that we could focus on granny.I have never experienced a more caring experience with a health care facility all of the front office staff were always so helpful when we would call in and check on granny. If the meals they were serving were not something that granny liked the dietary staff would have alternate meals they offered to her. The Managers from each department set up a little graduation of therapy ceremony with balloons & music they were all dancing around, clapping & singing as we left.The Therapy Department & Administrator gave granny a certificate for completing her therapy she insisted on framing it on the wall when she got home so she could remember how great the Brian Center of Clayton was to her. Unfortunately granny passed away due to health issues but up until her health & memory got to bad off she still remembered and talked about her stay at the Brian Center.The day of her funeral Ms. Mary even came out of her way on a Saturday to pay her respects to our family. She brought our family the most beautiful Memorial Bible personalized from the Brian Center Clayton.I could go on and on about how amazing our experience was. If i could give 1000 star rating i would with out a doubt Thank You all again from the bottoms of our hearts

Dont even deserve the 1 star but I will give that to Mrs Joanne thats it my Dad passed away and no one can tell us what happened the only person that kept us up to date was Mrs Joanne but everyone else needs to be fired they didnt even know a time my Dad had passed or what happened I have tried to talk to many people but it makes no difference i also called the 1800 number to make 4 complaints have heard nothing from anyone we didnt get all my Dads things which is crazy his stuff was sitting in a hallway no one knew where it was at I had to go looking myself very unprofessional if I would have had the say my Dad would have never been taken to this place all workers have pissy attitudes if you ask them anything and I want to know what happened to my Dad and I will find out some way because I am gonna call above the office heads until I get answers PLEASE DO NOT TAKE YOUR LOVED ONES TO THIS FACILITY they act all nice at the beginning then treat everyone bad after they just want the money also have seen how rough they are to other residents makes no sence how they swing them around and make them sit in poop for hours LAZY ASS PEOPLE YOU ALL WILL GET OLD AS WELL.... OUR ELDERLY DESERVE WAY BETTER food sucks also

My father in law was at this facility. He’s unable to walk or move himself . We visited him and had concerns the call bell stayed on for over 20 minutes and when someone finally came they had a nasty attitude and couldn’t answer any questions. The staff said she didn’t know anything she had just got here at 3:00 but it’s now almost 8:00 pm Then we asked to speak with his nurse and they NEVER came!!! I felt like my father in law was being neglected. He said no one had come to check on him!! I’m so disappointed in his care here at the Brian center!

Grandmother was here for rehab for a compression fracture. About 1 month in she suffered a medical emergency in the middle of the night. Bryan Center made no attempt to reach her family to aid in medical decision. They didn’t even go to the hospital with her. 86 year old patient with Dementia was all alone. By the time we found out she was transferred to a 2nd hospital due to her severity and in a coma. The first hospital would have been able to quickly and effectively treat her if they had her medical history from her family. I live 1/2 a mile from this hospital and could have been with her. She passed away. I will never be able to forgive the Bryan Center for not calling her family to be there to comfort her in her last moments of consciousness.

The worst place in the world to put your love ones. Terrible communication skillsNot professional at all.They staff needs to be retrained. And if you short staff, call your staff to help cover the shifts. No one takes their jobs serious anymore. Everyone is just there for a pay check. I trusted these people with my mother. Wont make that same mistake twiceHavent received as much as an apology. She was only there 2 weeks for rehab.

A nursing home was the last place I wanted to put my father, however with him needing 24/7 care and knowing some of the other facilities Brian Center was the one that I chose. We have had the best experience here and the nursing staff is awesome!

This is the absolute worst place to have your loved ones. I really hate that my mother was put through such a horrible experience. The staff was rude, the residents were rude (but I can certainly understand that), the food is inedible, and the place is just generally dirty and in terrible need of repairs. Mom was there 24 hours and had to wait no less than 15 - 25 minutes for someone to answer her call bell every time.Then once, when I went up to speak with the office people about an issue, I overheard the admissions person telling other staff members (very loudly) about when we arrived the previous afternoon. Most of the story was so embellished with sarcasm, I almost didn’t recognize it was us she was talking about until I heard her say my mom’s name. It took everything I had not to lose control, but I stepped in and confronted her about her remarks. The look of getting caught was priceless and the back peddling was fast and furious.I worked as quickly as I could to get her moved, but as soon as the staff found out she was moving to another facility, they treated her like she didn’t exist and one of the nurse assistants actually caused my mom quite a bit of pain.I wouldn’t let my worst enemy’s dog visit that place, much less ever have a loved one go there again!

Iheart felt helpless with no one to share my concerns when God sent me an angel. i pouted out myheartto her and she got the ball rolling to help to get each concerrns heard and addressed..I will return with my new assessment.on how effectively my cponcerns have been addressed.

This is the rudest and worst place to send your loved one. They are clueless! The administration and staff do not communicate. My brother in law was there after a stroke and he is not in his right mind. Rather than work with him to eat or take his meds, when he says no they simply walk away. In addition they pay no attention to his dietary needs and serve him food that he cannot swallow. They are awful and should be investigated and put out if business!

My dear, sweet, 92 yo mother-in-law was brought in late on August 29. 2017 and the charge nurse and CNA could not have been more kind to us. They checked on Miss Virginia every hour vs every two hours. The next night a new nurse came in to give her the pm meds. She talked to her and when she had finished the rxs, Miss Jo leaned over and kissed Miss Virginias forehead and told her not to worry, she would check on her again in a little while. We were overjoyed to see this special attention by Miss Jo. She is definitely in the right position and that night her act of kindness made an impression on us that we will never forget.God called Miss Virginia home the next day. We can say she was well taken care of in her final days and would recommend the Brian Center-Clayton to be trustworthy in taking care of your loved one.David and Lina FullerWillow Spring, NC
