I been coming to Oak forest Clinic Western Av Chicago Heights I like how they treat u got myself trasportacion cause I dont drive myself at all thats great for me cause that way I wont missed my Apoiments with my Dr Nancy Carter one time trasportacion they pick me up from the eye Dr Multack Olimpia fields Im very impressed with them very helpful with me I very happy

My husband and I were there for our Covid-19 vaccines. Initially for the first vaccine, there was a little wait, but they got the ball rolling and everything went quickly. The second vaccine was even quicker and no wait at all. The facility is modern, clean, and bright. All the staff was friendly, helpful, and professional. The facility is located in an easy to reach spot and has adequate parking. I would recommend them based on what I saw and the treatment that I received.

I felt very comfortable during my visit for the first dose. It wasnt crowded, there was ample space to be away from other people, I was greeted at the door by a staff member who asked identifying questions and told me next steps. I was escorted to the larger room where I waited only a short while for my turn; throughout my visit, there were several staff to explain what to expect and to answer questions. On my second visit, there was no one at the door.... a man who appeared to be security or at least an employee sat slumped in his chair, vaguely waved when I missed the green light to open the door and did not say anything as I stepped in. After a few minutes standing there in line, a woman at the long desk asked why I was there. when I said for my second dose, she said something like You have to go ...and nodded in the direction of the doorway...in that room. No one greeted me in the large room where many people were seated. There was room between them , but it was pretty full. ONE woman was giving shots, calling the next person, taking forms, handing out clipboards, making cards if a person forgot theirs, talking across the room to people who had questions...she was handling everything, but it felt really disorganized and completely short staffed. Performance for shot #1 = 5. Performance for shot #2 = 3 or 3 minus.

Everyone was great for the first shot. The second time I came, there was less organization. The people were NOT friendly. Did not make sure people kept distance. The receptionist at the front desk almost bordered on being rude. No one gave direction as to where to stand or where to go next. My husband was thinking of leaving his PCP and come to Oak street Health with me. After this experience, he is staying put. Its sad, but he was not made to feel welcome except by the Doctor who administered the injection.

The facility is very clean and welcoming. The nurse who administered the vaccine was kind, competent and totally on top of what was going on. I was in and out in 30 minutes. Great job!

Super high turnover rate leaving almost no continuity of care. Messed up bloodwork that they let fall through the cracks by not contacting me or treating me for. I had to pull up my results from quest diagnostics and ask about them when I came in for another appointment 2 months later. I did need medication. I Should have been on that medication months earlier. I finally switched to Riverside Healthcare and the treatment I have received there in the few weeks since the switch is night and day. I have a whole treatment tream there and they are amazing! They emailed AND called to speak with me about my results that night and as a couple more came in a few days later and took the time to explain it. This is not the kind of care I have ever received in my time (over a year, at least) at Oak Street Health.Also, Oak Street Health is an unorganized mess. I laughed when I got a handwritten card and post-it in the mail asking me to show up for a proposed reschedule appt since I missed mine seeing as how I called them weeks before that appointment and told them to cancel it and to take my entire file out of their system because I am no longer a patient and have moved elsewhere. Im not surprised to see none of that was done.If you are a younger patient, especially stay away! They say they cater to those on medicare, but I think they believe we are all elderly because that seems to be the patient group they are really aiming for. There are disabled younger adults! If you are on medicare, you can do waaaaay better than this place! Seriously!

HORRIBLE! Staff hasnt a clue.Nov., 2020: showed up on time for my appointment. Apparently, someone botched my appointment but NO ONE is allowed to tell me. My friend, who drove me there, sat in a cold car for over an hour, waiting. I sat inside, waiting. Finally, someone came out, took me in back and started to explain there was a scheduling error. I asked for an explanation, but we were interrupted by a lady who DEMANDED that my person leave imnediately. Weve been looking all over for you! The meeting has STARTED!!! My person jumped up, asked if she could be excused. Did NOT wait for my answer. Did NOT answer my question (would have taken her under 15 seconds).On the way out, I tried to reschedule. No one can do that except the person who just left me sitting, waiting for an explanation. Im DONE.

This 6 week long horrible experiment, was something my Medicare insurance agent said would be better for me with more benefits. It has been the worst experience with health care ever in life. It took a week and several calls to get my prescriptions called across the street to Wal-Mart. Ive been trying for 2 weeks to get an URGENT in network referal to a Urologist, only to find out the number given is to an Opthomologist office, and the address of the Urologist is not his. I still dont have a referal, and each time I call its a fresh hell that leads down another rabbit hole. Needless to say Im done with this whole medical network.

1st shot went smooth silk . Not a long wait . Moved through each station timely . 2nd shot not so well . Under staffed . Had one staffer doing everything and other staffers not helping . One patient even started helping nurse keep papers organized !

My husband Cecilio Torres Jr came in for an annual exam. So much concern for his pain medication for his brain injury that a prostate exam was not done!Here we are 2 1/5 months later and my husband is experiencing extreme pain and anxiety over his enlarged prostate! This is part of an annual exam for males. Not happy this was missed.Lost trust in this clinic.

The doctor is ok, but the time it takes to get a call back is terrible. Calls go to a call center and it takes 72 hours to get a call back. You cant even get a call through to the office. Not a good deal for seniors.

I went in for a surgery pre-qualification. It took several days to get through to make an appointment and then they told me my EKG showed signs that I had had a heart attack and I would need to see a cardiologist. After I got his clearance, it again was hard to get through and they told me they didnt get the report. Later, after I went into his office and watched them fax the report, they told me he hadnt cleared me which was untrue. The nurse simply didnt know how to read the report (my cardiologist said the same thing when I called)!!! They finally called and talked directly to Oak Street in order to clear up the confusion. This was Thursday and Im having surgery on Wed. I called and asked them to fax the report. They refused and said that I needed to come in on Friday b/4 they would clear me because they had to review the report with me.On Friday, they never showed up to pick me up and when I was called, they said I had to wait until Monday. I told them that wouldnt work because the clearance had to get to the surgeon 72 hours in advance!!! They finally agreed to pick me up.I waited 2 hours and had to ask 3x b/4 being taken to the back! When I was, they handed me the report without discussing it which meant there was no reason for me to come in!!!!!!!!!!!!!!!!! I asked to see a supervisor and after another 45 min., the nurse came in and met with me for 5 min. not really telling me and not explaining why I had to come in.I spent about 6 hours on the phone trying to straighten things out and at one point asked to speak to my doctor who NEVER called me back!!!!I am very disappointed because they advertise as specializing in seniors and our needs. I am seeking a new primary doctor.

Having to always go thru the associates is very frustrating. After FINALLY getting back to me about the prescription it’s not going to be filled ‼️‼️ I’m DONE
