I came here for a Covid test required for international travel. I paid cash. I let them know everything I needed to have on the test result document in order for it to be accepted for international travel including the name of the lab and the lab credentials. When I arrived to pick it up, the document did not contain this information. When I informed the person with short blonde hair at the front desk that I needed to have this information, she was extremely short with me. She told me that this is the document they give to everyone else for travel and that if I needed a more detailed document, I would need to call St.Dominics Lab myself to get the information. I paid for a service that they told me they could fulfill and they did not. I ended up calling St. Dominics Lab myself and having them fax over the full lab results including the name of the Lab and the CLIA number necessary for travel. This was very unprofessional as this is something that I should not have had to do.I personally believe that she did this because a) I am Black and because b) She could not appreciate my sweat shirt which reads God is a Black Woman

Went there for a drug test. I arrived an hour early. Waited an hour. I asked the nursed in a casual manner what took so long? Because I arrived before a lady and she went before me, and I was just curious. In a kind of rude and defensive way she said its only one nurse and doctor here right now and the other was already in our systems. Then I told her I was about to be late for work, and all she said was thats a time management issue. I dont understand the statement because the whole point of mr arriving when the clinic first open up so I wont be late. Terrible customer service.
