Moving a senior citizen into an assisted living facility is difficult even in the best of times however, during the covid-19 epidemic, it was a more daunting task. The staff of Oak Park place was wonderful! They were easy to work with and willing to do everything they could to make a move at this time go very smoothly. They have helped to make transition very easy and have always been extremely professional.. At a difficult time, their kindness was very much appreciated!!

I was so impressed and immediately felt comfortable with Debbie and her team. Although my loved ones chose to go back to their home state and not take the next step to live at Oak Park, Debbie spent many hours navigating me through an extremely difficult time in our life. She explained various procedures and how to seek health care opportunities ultimately allowing for difficult discussions. I would recommend Debbie and her team at Oak Park as she truly cares for her seniors.

My Mom, sister, & I have been to Oak Park Place about six times to seedifferent entertainers. We have never been disappointed. The staff has always been friendly & very welcoming and the talent of the performers wonderful. We sure missed attending these shows because of Covid,but were looking forward to seeing some shows in the near future. I have a friend who is in memory care. This is a beautiful place for anyone who has these needs. It has taken my friend awhile to get settled in but the staff has really helped, she now plays bingo, triva, loves going out on the patio. She gets her hair done every week and volunteers paint her nails and alot more activities. The staff is great and always meets her needs and the family. I would certainly give Oak Parl a 5 star.

This looks like a wonderful facility. I was impressed with the attention to detail in the design of the accomodations, from the non-slip floors to the easy-opening windows.

The people at Oak Park Place in Burlington went above and beyond to help me with finding a place for my 96 year old father. I happened to stop in on a Saturday to see if I could just pick up some information. Heidi offered to give me a tour. Oak Park Place is a beautiful facility. They have so much to offer. Everyone was so friendly. Heidi, the director of housing, knew all of the people we walked past by name. A very kindly resident allowed us in his apartment so I could see what it was like. He told me to tell my dad that Oak Park Place is a great place to live. Heidi gave all my information to Debbie Waldron-Smith, the director of sales. Debbie followed up with me several times. She offered all kinds of options and solutions. I would highly recommend checking in to Oak Park Place if you are in need of assisted living or memory care.

I have done home health care in this facility for years and have always found staff to be cooperative and demonstrate good carry over with my recommendations. I also visit residents there and they are quite satisfied with the facility. Debbie Waldron-Smith is really amazing as the bridge for a resident to move from home to assisted living. The staff at Oak Park Place helped our family at one of the most difficult times. Deciding that it was time for my Dad to go to memory care was a hard enough decision, much less the actual transition, especially during COVID. Debbie walked me through the process, both physically and emotionally, she was available to answer any questions. She was very patient and understanding with my Dad and our family members. They have worked with us so that my Dad can have walking/activity visits with his best friend, his dog at least twice a week. Its good to see a smile on my Dads face as hes transitioning to the next chapter of his life.

My mom is content and comfortable at OPP memory care and I couldnt ask for more. The staff are friendly and kind. I returned for a visit after a year and my mom was neat and clean and so was her room. The prior year, although she visited with others briefly, she often kept more to her room. This visit she was chatting with 8 others in the sitting room and when I left her room briefly there she went again to visit. Awesome!One more thing...they let my mom bring her piano to the activity room on her floor and it has been wonderful for her and for others.Thank you OPP!And thank you for everything you do every day and especially this past year!

I have about three years experience with Oak Park Place as a volunteer. A good old friend became a resident there and invited me (his former pastor) to work with their activities’ director to develop a weekly half hour non-denominational worship service. The cooperation and support from OPP was very positive and helpful. In that capacity I saw weekly encouraging, compassionate care from OPP staff for the residents who expressed appreciation for the care.

When we looked for a facility that met our needs such as comfort, safety, friendship and enjoyment. We found these needs at Oak Park Place. We also found the compassion given to us from the residents and staff welcoming us to our new home. My mother is a current resident in memory care at Oak Park Place and my father was in assisted living prior to passing away in the fall of 2020. Both of my parents have received excellent care throughout their stay. When my fathers health declined, the staffs extra time and attention to his care was enormously comforting given his prognosis. I have had several experiences with Oak Park Place in the last 5 years. Unfortunately, it was not built until after 2014 when my own Mother passed away because Oak Park would definitely have been my choice for her to live.My first experience was in helping my sister-in-law move her Mom and Dad in and out and the people were so kind. Following that time, I began a senior citizen program at my church. i reached out to Debbie and staff at Oak Park and together we formed socials at our parish sponsored by them, and she kept me informed of monthly events at Oak Park Place. Since then, we have been in touch regarding our parish and non-parish members to see what their needs might be. Debbie and staff do a wonderful job with their residents and their families. The facility is immaculate and not-to-mention, beautifully decorated. I would highly recommend Oak Park Place. It is a wonderful addition to the Burlington area.

My mother needed assistance when she broke her hip. Oak Park was there for her when she needed that extra help. She has been a resident for 3 years now and still is very happy there.

When our singing group came to sing for the lovely people at Oak Park Place Burlington we were welcomed with smiles and good wishes by the staff. The residents were very appreciative of our singing, joining in and asking for more. It was a blessing for us to be there with them. We are hoping to be able to come again one day to this clean, well kept facility.

Both my parents live at Oak Park Place Burlington. My dad lives in an assisted living apartment and my mom is in memory care. Both are very well cared for. My dad is able to visit my mom as often as he wishes and the staff has been exceptional in their flexibility with this arrangement. I am secure in knowing that their health and safety is very important to all staff.

Oakpark Place is a great facility for my mother! After she fell and broke her hip she needed more care and has adjusted very well to life here. She has a beautiful room and extra care available when she needs it. She is very content here, even throughout the long days of COVID.

I work for Oak Ridge Care Center and we have partnered with OPP several times in the past, transitioning residents from our care into theirs. Debbie and all other staff I have had contact with truly care about their residents wellbeing and take pride in providing a place that feels like home.

We went to their Veterans Day program they put on for the veteran residence. It was well done and showed they really care about the well-being of the residence.

Even though our family will not be able to do private pay, I got so much useful information about services and agencies that may be able to help our family get care for our parent. They were also very knowledgeable about VA benefits available.

I have been working with Oak Park Place in Burlington for a few years. This was to be my Dad’s next home when he was ready to take the next steps in his Alzheimer’s care. I watched the structure build daily on my work commute. I toured with the staff and knew this is where I wanted my father to call home and be cared for. Unfortunately, with 2020 and COVID, my urgent need to place Dad was not possible due to availability. This did not stop Debbie Waldron Smith. She gave me more resources to use in her network of contacts. Where the Aurora team dropped the ball over 3 days ago, Debbie picked up in less then an hour. I now have home services and Dads new home in the process. Thank you Debbie for stepping out of the box and your compassion for people.

I have been working with staff at Oak Park Place for over a year now. They truly care about their residents and their families. They are very compassionate and share the good times and bad with their residents and their families.

I have been coming into Oak Park Place for about 6 years. I am a home care Physical Therapist and work one on one with residents. I have found the staff to be very helpful and friendly. The building is always clean. While I have never eaten there, I have been told by the residents I work with that the food is very good.

My father and mother moved into Oak Park Place in January 2018. From the moment we toured the place we knew this was more than a cookie cutter assisted living home. This was a place that really felt like home. A place my parents enjoyed moving into.We had experienced other facilities and I can honestly say the staff at Oak Park in Burlington is the best of the best. They are all so loving and caring. My mother adored all of her caregivers!My father passed shortly after they moved in however, my mother thrived after his death because of the staff and the wonderful friends she made in the other residents! Every day they have engaging activities to participate in. My mother enjoyed being a resident ambassador and showing others the great reasons to move to OPP. Unfortunately, she had to give that up as her health worsened. Although her health was declining she still found joy in every day thanks to the wonderful place she found to call home.At the end of her life the dignity, compassion and love she received from the staff was amazing! She left in comfort and peace thanks to the fantastic staff.If you are looking for an assisted living for your parents. I highly recommend Oak Park Place!OPP #amazing

My mother just moved into Oak Park Place. Because of the restrictions for Covid, it could have been a problem. But, the Oak Park staff stepped in to make the move flawless. In particular, since family is not allowed in the facility due to the Covid restrictions, they acted like family to get her all set up in her apartment.

What a wonderful place to live and work! I personally come to the building several times a week to for my job and everyone there is great to work with. The floor staff, Director of Nursing, and all administrative staff truly care about their residents and are knowledgeable it the roles they work! By far, the best community in the area for any loved one!

As a former Administrator, I have the utmost respect for the staff at Oak Park Place Burlington. As you walk through the beautiful home like setting, one is surrounded by a team of warm and welcoming professionals; and it is always bustling w/engaging activity for its residents who always seem to have a smile on their faces. That is a direct result of the caring environment created by the staff; from Debbie in Marketing, to Peggy at the reception desk, to Heidi the Executive Director and Chelsea the Director of Nursing and her staff. No matter what stage in their life a resident is, the setting provides dignity and honors the individual for not only where theyre at now, but where theyve been. I have the privilege of witnessing this each time I visit; and those visits are marked by the show of respect to those who have paved the way for all of us.

Although mom wasnt a fit for Oak Park Place, Chelsea was absolutely amazing. She continued to help me ensure my mother would receive the care that she needed. Ensuring an aging parent is safe and taken care of was our priority and Chelsea made it happen. The whole team was wonderful. What a great addition to the community. I love love love Oak Park Place. My mom entered pre-COVID and the activities were awesome! They have done a PHENOMENAL job keeping her healthy and safe!!!!!! My mom sends me pictures nearly everyday of her meals. She says it’s like eating at a 4 star restaurant for every meal. Also, it’s very clear that the workers care deeply for all of the residents, their safety and well-being. Thank you Oak Park Place!!! We couldn’t do this without you!!

Assistant director is very unprofessional ( terry) I was supposed to have a interview but I had been exposed to someone with covid so I let her know I couldn’t come in due to that and that I wanted to get tested. She had insulted me by saying I’m unreliable . How am I unreliable for not wanting to spread covid and get others sick especially where there are old people . It’s very unprofessional how she handled the situation and how she was willing to risk families loved ones getting sick . Definitely not a place I would want a loved one to stay at . Stay safe and be careful of this place.

I have nothing but positive things to say about the Oak Park Assisted Living/Memory Care facility in Burlington, Wisconsin. From the first point of contact via phone, to the welcoming my wife and I received the first visit while looking for a new home for my father-in-law, to the actual move in day, the staff was exceptional! Very caring and understanding to our needs and really bent over backwards for us in every way. The facility is beautiful and well taken care of. Its like walking into a fine hotel and also feels very comfortable and not overwhelming. Each and every visit we are greeted at the front desk by friendly staff and when asked, they are able to give an update on how things are going. My wife and I find just that fact alone very comforting knowing that our loved one is being cared for properly. Within the Memory Care area, the staff is also exceptional and are always available to answer questions or just to have conversation with. The staff are always smiling and interacting in a positive way with the residents. A full day of activities are available for the residents and the meals are wonderful. (Note: Oak Park Place mails out each month a news letter to inform the family of daily and special events taking place at the facility) The common areas within Memory Care are outstanding and it truly feels like home. The individual resident rooms are roomy and have the ability to personalize making the transition even easier. I would recommend Oak Park Place Burlington to anyone looking for a caring, well kept facility for your aging loved ones needing that special care we all hope for.

My aunt lives at oak Park place and received phenomenal care!! She was a challenging patient suffering from Alzheimers but the staff was patient with her and took great care of meeting her needs.

Finding a place for mom or dad is always a difficult task. Our experience with Oak Park Place was excellent. The staff is wonderful, caring, and always willing to help. It was such a delight to find that family members were able to bring in their dogs and puppies to visit the residents as well. The facility is beautiful and always very well maintained. Truly a five star Place!

Placing our Dad at Oak Park was probably the most difficult decision me, my siblings and our Mom ever had to make. We didn’t know what to expect and how this would turn out. We soon realized that Oak Park was the best place for Dad, considering he couldn’t be at home anymore. He received excellent care from the kindest caregivers who were always friendly and had smiles on their faces. They were attentive and patient with Dad and us. They were encouraging but not pushy and we were kept informed and updated regularly. The facility is very tastefully designed and decorated and comfortable. Dad always said the food was really good! Dad is now in his forever home with the Lord, and we will always be grateful to Oak Park for taking such good care of him in his last days.

Making the decision to move my mom into Memory Care 3 months ago was one of the hardest decisions but Oak Park Place went above and beyond to transition my mom into her “apt” quickly and smoothly when we needed help immediately. Debbie was there with me through each step and was wonderful! All the staff have been wonderful! My mom tells me she loves her new “apt” and she’s happy so that makes me happy. I can sleep at night knowing she’s being taken care of and is in a safe place ❤️

Our mother was at the Burlington Oak Park Place for 8 months before she passed away. She enjoyed her living experience and felt the staff treated her like family. The were responsive to her needs and as a family we appreciated how the staff attended to her needs. We are grateful for their care.

Our family is very pleased with Oak Park Place. As a family we are able to come and play card games especially Sheepshead in the Bistro, as long as we say when we would like to have the room for our family the provide it. It is great to be able to bring the family to our loved ones staying at Oak Park Place so they can continue to participate. We enjoy Family Feud and Bingo also. Oak Park does numerous things to bring the families together and keep them together as much as possible. We really enjoy being there.

Our dad has been in memory care at Oak Park Place for nearly a year. We are 100% satisfied with the care and attention he gets there. The staff is very pleasant and attentive to all his needs. We have total peace of mind knowing that he is in good hands and we do not need to worry about the care that he is receiving there.

I have been to OPP multiple times, the first being to attend a free presentation to learn about Power of Attorney and Living wills. Each time I visit I am overwhelmed by the positive, inviting atmosphere within this “community”. The leadership team communicates so respectfully with the residents, and they are always available to assist and answer any questions.I witnessed residents enjoying a trivia game that was being lead by staff, and the leadership (Deb) mentioned that there are numerous staff-run activities to engage the residents. The leadership team displays an such an encouraging environment, it seems to be contagious to both the residents and staff. The warm-hearted feeling I get when I visit is because of the welcoming environment of the leadership team.

This beautiful facility has staff that are very loving to the residents. Everyone is friendly and helpful. The residents seem very happy and feel like they are at home.

I visit patients at Oak Park Place regularly to service their hearing aids, and I have not experienced a nicer environment for residents requiring varying degrees of assistance. The facility is beautiful, but what really sets it apart is the staff. Care providers, nurses, reception, activities, sales, management, food service, maintenance are all top notch. I always recommend people visit Oak Park Place when looking for any type of assisted living.

Oak Park Place did an excellent job caring for my mom during the Covid lockdowns. I am impressed by their record during this challenging time. Now that we can visit again, they continue to maintain high standards. Mom previously lived in a “highly-rated” facility in a nearby town for three years. Our family decided to move her to OPP in Oct.2018 and we are so glad we did—better management, better honest and caring staff, and more programming. Mom especially appreciates the religious services and the daily exercise program.

Mom has been at OPP for almost a year now. It took some time to adjust to not living at home, but she has settled in now and loves it. She has gained weight(she was not eating well at home) and participates in almost all the activities. It is nice to see her make new friends, including the staff members, at age 97. Oak Park offers a wide range of community programs, on site activities and rehabilitation with Home Care therapists coming to the patients right there in their apartments!
