I know many people who went here , one being my Father . And a few people who worked here . One for 15 years . They treat people who enter Very Well , And are AlwaysFriendly & Respectful

They are the worst place DONT ever send your love ones there. They are rude they leave the patient in urine or several hours on the bedpan. they dont feed you appropriate food or its cold. and its supposable under new management revaluations same disappointment.

I am happy to say that my stay at the Guardian Center during the months of December 2020 and January 2021 was wonderful. I was cared for in a totally professional way. The entire staff was alert to my needs. The PT staff taught me to walk again! It was excellent.I give it an A rating. Many thanks! PS Donna C is the greatest communicator with my family. I came here for rehab on my foot.Everyone from the nurses to the CNAs are excellent. The PT and OT have been excellent also. The only bad thing I can say is that the food is horrible, but there going through a change of ownership so hopefully the food will get better. Ive been at the Brockton facility for about a month altogether right now, I highly recommend it.

After a long stay at the hospital everyone at guardian went over and above to get me settled in and back home. This was my first time at a rehab and all my expectations were met. Well educated staff made me feel comfortable and safe. I learned a lot with therapy and would highly recommend Guardian

My mother spent a couple of weeks at The Guardian Center after her knee surgery in 2019. I’ve never seen more attentive and caring staff - everyone was so kind. There was always a nurse or aide available if mom every needed anything. Mom’s room and the rest of the facility was always clean and well-maintained. If your loved one ever needs extra care (long term or after a medical procedure), I highly recommend The Guardian Center!

Absolutely horrible. I would never recommend this place. Worst communication I have ever seen. From the day my father was admitted to the day he passed away there a week later we could not get a phone call back from anyone. We were given numbers to call or text with any questions or concerns and to this day still have never received a response. The day he passed I called and was hung up on multiple times. I think I called 3 times before someone answered and said he died. I picked up his belongings today and again didnt get a call back about paperwork and instead still continuing to get the run around. You would think him passing under their watch they would have handled the situation a lot better. We will make sure no other family ever has to treated and put through what we were.

My mother has been a resident and patient at the guardian center for awhile. I am very appreciative of the care given to her by the staff. I have always been able to communicate with the nursing team regarding any issues or concerns that I or my family members have. The Information ,level of care , treatment and timely responses during the covid-19 crisis has been amazing

My mother was a patient at the Guardian Center very recently. She and our family members received wonderful compassionate care from all the staff in all departments. The communication was professional and timely. We always knew what the different departments were working on with her for plans and goals. She was able to her home in Bridgewater with some services from the local VNA. Placing a Loved one in a nursing center is a difficult decision. I am glad that the Guardian Center was there for my motherduring that difficult time.

The director of nursing and the doctors that treat patients did a miraculous job with my 87 year old mother. After many months and changes in meds ordered by her now previous doctor my mom was in a constant panic mode and has dementia. Her previous doctor would not put her in the hospital for observation and finding the right mix of meds to help her. At a loss of what to go we put her in the guardian for respite care privately paying and hoping they could help her. They did a remarkable job. We were ready to send her to a live in facility. Yes she still has memory loss but she is like a different person, calm during the day and sleeping through the night. Staff is kind and considerate. Thank you to all of the guardian staff and the director of nursing Sherrie

ALL STAFF PROFESSIONAL, COURTEOUS, KNOWLEDGEABLE.FACILITY CLEAN , WELL KEPT, LARGE ROOMS , WINDOWS OPEN ABOUT 1 FOOT, BEAUTIFUL WOODEN FLOORS, BRIGHT PAINT , DECOR , SOACIOUS DINING /FUNCTION ROOM.NEVER A NURSING FACILITY ODOR , I.E. URINE , FECES.THE FOOD PORTIONS ARE SMALL. FOOD QUALITY IS FAIR. THE ADMINISTRATOR IS AWARE AND TRYING TO IMPROVE.I WOULD RECOMMEND A STAY HERE FOR FAMILY, FRIENDS.

I had my mom in Blues Hills of Stoughton last year ... when we had a bad experience and i even contacted the Board of Health and one issue was taken care of but everytime we walked in we would get nasty looks & nasty attitude from ALL OF THE STAFF - a friend of the family recommended the Guardian ... we went to talk with the staff and they were awesome .. i even told the administrator that i would call back to set up a date for a tour .. she then said nope i will give you one now ... she showed us around and even took me out back to the DEM/ALZ area where my mom would be ... the minute we walked out back the nurses were so welcoming ... then the administrator asked me to send or come by with all my moms paperwork .. i explained why i had to get her out of stoughton .. within 6-10wks my mom was transfered and it was the BEST decision i had made ... they are very welcoming -clean - polite and the day she was transferred i call and told them that were she was being brought over to brockton but it was lunch time and she was getting hungry and when she arrived they had brought her out a hot meal then showed us her room ... compared to Blue Hills they keep it very clean ... and if i or my mom needs anything its done without hesitation and my mom is more comfortable here then she was over in stoughton ... ... so yes i would recommend the Guardian to anyone ... so please dont turn around and walk away just because of one bad review ... cause everyone experiences things differently ... I recently volunteered here through my school and I had a wonderful experience! Everyone was friendly and helped teach me what I needed to know. The Guardian Center is a clean, peaceful place for both the residents and the staff. Whether you are looking to volunteer or get a job here, I would highly recommend!

My mom stayed at this center on 2 separate occasions recently. She was given very good care. The staff was attentive and very helpful. Our family is very grateful for the hard work of their Staff, CNAs, and Nurses. We want to give a special thank you to one particular CNA named Sherry who works on the 3rd floor of this facility. What a great person she is. Shes a joy to be around and she makes sure she treats everyone with respect. Many Many thanks to you all for taking such good care of my mom! Youre appreciated!

I found it very clean with no odors, the food was good and the staff was very kind.The activity department did awesome projects with the residents and showed respect and warmth to everyone they worked with!

My aunt was on the dementia unit. It was less than impressive on many fronts, but the stenchOf urine and feces as you walked around the unit was horrendous on some days. They left the dirty laundry baskets in the halls fill with waste. The unit was dreary and dated. My aunt was in a remoteArea far from view of the common area and msg station. Her room looked like it hadn’t been updated since the 50’s. Dismal. No one looked happy, staff or patients. Configuration is not conducive to safety.Positive: Food was pretty good and a particular OT staff was very good and kind.

Went there for an interview, it was the worst mistake . The staff werent prepared with any questions, staff walked in the room talking on the phone while the interview taking place, others came in and reaching over me for food and asked the most ridiculous questions. Then after that Im told the job is $13/hr. The manager told me if i get a call then good, if I dont then SORRY yeah she said it in a condescending way. The work load put forward...they really expect to pay someone $13. They didnt disclose the salary prior to me going there. One of the Activity Assistant was VERY RUDE and DISRESPECTFUL with the way she handle the session. The place was a mess...was turned off the moment I walked in.
