Northwoods takes such good care of my father. The pandemic has made it so difficult, but they protect the ones they serve, as well as their employees. They keep families update with with text messages. Everyone is so kind when I call in to check on my father. Thank you so much!

Ill sum things in a few sentences. Since my father in law Milton Prices passing my wife and I have been back to Northwoods to thank a couple of staff members personally.1, joe the night nurse probably does a goodjob but is very confrontational.2, Nurse Mark is the one who called theDoctor because he noticed Miltonsrespiratory problems3, In physical therapy Mark and Maria wereawesome caring and cheerful people. Milthad a hard time in therapy but lookedforward to spending time with thetherapist.4, As far as anything else at Northwoods its amatter opinion. The whole place couldstand an update in decor.But overall patient care is good with one exception and this one is totally unacceptable.A aid with black hair and bright pink highlights walks around looking busy but does nothing. On numerous occasions we observed her walking into rooms turning off the call light and walking out never to return to that patient. I tell you in the 2.5 weeks my father in law was in your facility she did this on several occasions. But only once in Miltons room.Feel free to contact me,Respectfully Reinhard Pilz

Very friendly staff and very professional.

I have worked for Northwoods for almost 9 months now and I couldnt be happier that I made the choice to switch to this facility. Having worked in long term care for almost 20 years, Northwoods has been one of the best places I have been blessed to be employed by. The staff are all very caring and compassionate and are always so eager to go above and beyond to make any and all guests feel like a priority. I would recommend Symphony Northwoods to anyone looking for short term rehab and/or long term placement for a loved one

I am in love with working at Northwoods! I love the patients. I definitely love the staff. I am miserable right now having to part from them. Not being able to arrive on time effects my patients and co-workers 😔 I am extremely saddened to leave. I would go back in a heartbeat 🥰🥰🥰... At this time the available shifts are:1st 6-2, 2nd 2-10, and 3rd is 10-6am. I would need a shift beginning after 7:30 am. The pay is competitive. I suggest you check it out! Its small, quaint, and friendly.😍

I would highly recommend Northwoods Care Centre. The staff was highly knowledgeable and attentive with addressing all of my concerns. The environment is healing and made to feel like home. I will miss the team here, thank you for exceptional care!

Northwoods is a great place for your relatives to spend their last days as well as being treated with respect care and love

Ive worked with the Maple Crest and Northwoods teams going back to 2012, and Ive enjoyed our collaborative partnership.The leadership has been consistent and focused on advocating for its patients, and dedication to quality of care. Northwoods CMS 5 Star rating is no surprise and I would recommend their locations to a loved one or those in need of a SNF.

WOW! What a difference! We considered and toured this facility a few years back and decided to go somewhere else. Gave them a try this year after a grueling stay in the hospital and I cant say enough about the improvements that have been made. Very very pleased with the new, clean, and inviting atmosphere and extraordinary staff. Im picky too! So if youre unsure, trust me and give them a try.

I have worked at northwoods for 30 plus years. I love the residents and families, they become like my extended family. I enjoy the relationships I have with them. My co-workers are also like family, the team work is phenomenal. Not only do I work at NORTHWOODS but my grandfather and mother were also patients here and the care they received was top notch.

I have been working at Northwoods for 13 years, I am a single mom of three, and Northwoods became my second home. Northwoods not only cares about their patients, they care about their personnel and their families. I started as a nursing assistant and Northwood has giving me the opportunity to succeed in the professional field. I am very happy Northwoods gave me the opportunity to work for their company.

If I could I would give it negative 10 stars. I stayed there to learn how to walk again. When I hit the call button I would be waiting for more than an hour sometimes. Then when I had to be changed I would get an attitude from the workers. Then when it came to my meds they messed up twice then I was told I had to remind them of medication to give me before meals. I have to change rooms because my roommate was Threatening My life and was to horrible so I need to be moved. The food was horrible we got cold meals for dinner and half the time I didn’t even pick what was given to me because they filled out my menu instead. All and all this place was horrible. I wouldNever send anyone here. I told any of the workers who would listen

My mom has been here since November. I cannot say enough about the care& concern the staff has shown her. Unfortunately my memory doesnt recall all the names but one in particular has gone beyond the extra mile. Her name is Whittney. She has been the angel in some very tuff times for my mom. Today, she was my angel. Without me even asking, she reached out to say she would make sure my mom is there for her granddaughters Senior year Awards banquets & her graduation. My mom is w/c bound so Whittney went the extra mile to figure it out. You are the BEST Whittney! You have no idea how much this means to me, my daughter & my mom! Northwoods is lucky to have you as well as all the staff. My mom is very blessed to have all of you.

My experience was pretty good. The staff was aware of all my needs and were attentive to each of them. There were a few small issues i raised and when brought to their attention, were handled immediately. The only complaint i can find here was the food. Every meal was tasteless and poor quality. I had my family bring me food almost daily otherwise i would have probably lost 20# had it not been for them. I think they need to find a better food supplier. Having worked in the food industry myself, i know there are better vendors than the ones they use. Quite honestly, ive never seen so much food go to waste than here. I think in proving my point the corporate officers should be brought in to try to eat a few of these meals themselves.

Terrible, Terrible place to bring your loved one. Staff is awful, irresponsible, not compassionate, no empathy. No communication whatsoever. My Dad was put in this place after a fall that left him with fractured ribs and fractured wrist, on night 2 of his stay he fell out of his bed and tore up is shin from knee down and we just found out he broke his toes also. He is diabetic so any kind of injury is severe. Took him out after 5 days will never go back. I wish I knew who to report this facility too.

Ok so Im not saying this is a bad place they made sure my grandfather was taken care of but the problem is they dont communicate very well I have called there 6 times today no one has called me back and my grandfather is no longer at the facility and they still continued to collect his monthly payment and now I cant get no one to call me back to be able to get my grandfathers money back

A great facility with a caring staff.

My grandpa stayed in this facility. We had him out in less than a week. He hated it and talks about how horrible the experience was. He had to stay on the toilet for long periods of time because no one would come back to help him off. He said it was very painful to sit there for so long. He urinated in bed because no one would come when he pushed the call button. My cousin found a bed sore on him and noone that worked at the facility had even found it or treated it. He was only given some of his meds if a certain CNA was there, otherwise they just would not give it to him. Also my mother called with some concerns and no one ever called her back to address them. They only gave him 1 shower after he had been there almost a week. After my mom complained they finally gave him one. Then after we told them that we were taking him home they never even sent an order for meds for him to go home with. I would strongly encourage anyone looking for help with a loved one avoid this place!

Exceptional care and quality service!

I worked at this facility. This facility commits medicare fraud with a certain resident while she is waiting to go to another assisted living, always changing her diagnosis to keep her in a private room. She is a family member of several staff members. Michelle Kohn RN allowed Kevin Richards CNA to bend a residents fingers back and never reported it to administration, nor the state of Illinois as required for abuse of the elderly. Michelle was also witnessed harassing a patient about his own electric wheelchair, and told him he cant have it, he does not have any rights to use his chair. This nurse would also force medications down a dying patients throat. This same NUrse charted several days in a row a full body assessment was completed, but another facility found a weeks worth of patches still attached to the resident. Mariella Feliciano RN has allowed numerous residents to fall and never completes the incident reports. She also allowed Maribel Carreno CNA to drop a patient in the shower room without using the proper lift equipment. This nurse also did not react fast enough and allowed a resident to pass due to a heart attack. This Nurse also allowed two residents to be battered by another resident on two separate occasions, not once ever calling the family. Diane Stewart CNA charts all the time, everyday, that ADLs are performed on all residents. Never once saw the ADLs being performed on any resident. Kevin Richards CNA will always leave residents soaked, takes them to the bathroom, then pulls up the same soaked brief back onto the resident. Dorothy Polcher CNA pushes residents into rooms and turns on the call light and runs to smoke in the shack. She was caught in the act several times, never once did administrator Ben Esguerra do any type of reprimand. Mario CNA, on second shift will allow residents to sit in their own stool for hours. Administrator Ben, states since he gets complaints about these residents he no longer has to care for those residents. Amanda Barona RN orders the CNAs to falsify all charting so the facility qualifies for more funding from Medicare. Bob Novak has hidden mold in room 119 behind the wallpaper. Lia Hendricks CNA is performing Nursing duties as a CNA with the permission of the Nurses at night because she is friends with Ben Esguerra and Rachel Quilal-lan, she is not licensed in the United States. On Multiple occassions this CNA has verbally abused and neglected residents during the night without changing dirty linens. These situations have been reported to Administration since this CNA is friends no discipline has ever taken place towards this CNA.
