My mother in law lived at Augustana for 13 years. I wanted to take a moment and write just how much I appreciate the care she had. She was very happy and met many friends during her time.

This is not the place to bring your love ones, the staff are lazy dont care about the people its all about the money.I was here for 5 days the worst experience ever I wouldnt recommend this place if it was up to me.Very unprofessional and unorganized.Rude you name it.

I came straight from the hospital to Augustana Care Health and Rehabilitation after one week with a rare syndrome. I was weak and a ParaplegicWithout the talent, attentiveness, love and assistance by my aides and nurses, my days would have been miserable. They gave me joy every day and met all of my needs.The business office answered all of my billing questions and the health information staff provided the proper documentation and helped set up transportation.Upper management would listen to concerns and suggestions.Physical Therapy and Occupational Therapy were provided once a day byCentrex. Management and therapists were stellar! Without my therapists providing passive and functional therapy, I would not be walking today and ready to return to my home.A myriad of fun activities are provided by a caring and fun group.The meals are excellent and are provided in your room or a dining room.Laundry is done daily by a smiling staff with great deftness. Maintenance is always available and do an exceptional job. Housekeeping keeps the facility immaculate,There are very comfortable sun rooms, a lovely gift shop and patio. The surrounding areas are beautiful with trees, a pond, gazebo and convenient parking.I would recommend this center to anyone and I thank everyone at this fine establishment.

This nursing home killed my mom. She was living in Augustana since July 2016. In October 2016, I stopped by visiting my mom after work, she got redness and swollen on her right face, arm, and leg. I immediately called nurse over, the nurse measure her blood pressure it was 180. I was shock then I requested that she needed going to ER. Once, my mom arrived at ER, she took the CAT and MRI tests. They found out my mom had a silence stroke in her brain. In Sept. 2018, one night my mom got up and went to bathroom, then she hit her head by the wall where they hung the box for used needles. Because the next morning I came over picking her up for eye doctor appointment, I saw her left face and eye bruised so bad and the left side of her head had a bump. That was what I had told. I went to nurse station let them know. It seemed nobody had responding for that. Then in October 2018, after work I stopped by to visit my mom. I saw blood was on her toes and the walker. As I asked her how come, she told me she was bleeding in bathroom. Then I went in there to take a look. There was a lot of blood splashing all over around the sink and at the bottom of the wall underneath the sink. I did glance at the trash can, and I also saw blood in it. Nobody got noticed from that. I had told that the janitor who was the one saw the blood and cleaned it. Then I ran out of her room to find the janitor and asked her. Yes, she said she saw 2 soaking blood of towels in the trash can as she cleaned up that morning. I was so up set and I asked the janitor that she did report it. She said she did. But I didnt get any call from the nurses. The nurses whoever worked on night shifts. They have no responding for residents at all. After those situations I came to visit my mom more often liked every other days. The last day I saw my mom was on Wed. January 9, 2019, I stopped by after work. I brought dinner over to feed her that evening. My mom and I were sitting together a couple hours. Everything seemed okay for her and she was very happy on that evening. Then afternoon around 2 pm of Friday January 11, 2019 I got a call that my mom had no responding for lunch. As I came over, she couldnt talk and hold my hand anymore. All the staff didnt know what happen to her on that Friday, January 11 morning. Nobody knew what happened to my mom on that morning OR the night before on Thursday January 10, 2019. I asked them did my mom still okay to have breakfast on the morning of Friday, January 11, 2019. Nobody could answer my question. I got no phone calls from them on those days. When I mention that with nurse manager, she said they called me. I did check on my home phone and cell phone as I didnt receive any message from Augustana Nursing Home on those days. That was a big liar. It was broken my heart ... Then my mom passed away a week later on Saturday, January 19, 2019. My mom lived in Augustana Nursing Home as long terms care of resident. Every month, the State of Minnesota and Federal had to pay for nursing home arrange from $6,000 - $7,000 of the cares for my mom. Unfortunately, nobody at Augustana was care about my mom.

My mom has been to transitional care (rehab) five times in the last few years. We have always had a fantastic experience. The nurses and aides all knew her and were always happy to help with anything. She had favorite nurses (Stephanie, Solomon), but they all treated her in a loving manner. She passed away last month and one of her favorite aides was holding her hand when she passed. It gave our family comfort to know this. They were nothing but kind and respectful.

I worked here, two different locations. Hastings and Apple valley. These facilities are a joke. If you want your family properly cared for don’t bring them here! We constantly would work short, making it harder to provide the care our residents needed. You have to badger the nurses to get help for med requests from residents NURSES are LAZY. I was feeding a resident who couldn’t feed themselves in the dining room, and got called to the nurse station for a call light, nurse came to retrieve me for a resident asking to sit up in bed! Something that would’ve taken 2 seconds to help them with. Some CNAS leave the residents in the same clothes for a week. Nothing is in order here, everyone runs around not knowing what to do. One CNA is expected to care for up to 15 or even more people making you hate your job because you can’t do the job you were hired to do. I was fired because I was exhausted working short, and didn’t document that day. I actually cared for my residents, made sure they were properly cared for. Unlike some CNAS who bring their resident to the dining room naked (under a sheet) for convenience of showering afterwards, not brushing their hair, or letting the residents have any sort of dignity. These people are treated worse than animals. Please do your family member a favor and look elsewhere.

My mother had a stay at Augustana Care Apple Valley. The staff was attentive, caring and professional. She was able to transition to an assisted living facility. We would recommend this care center.

I wish there was something less than a one star. Not only did I have a horrible experience. Couldnt find my pain meds that I hand carried the scrip from St. Joseph hospital to them. The food was totally unacceptable to people that have any kind of restrictions in their diet. Hot dogs, macaroni and cheese, watermelon that was mushy and coffee for dinner. My husband brought my dinner because there was nothing on the tray that my doctors allowed me to eat. I ask for a cup of tea but it never came. I also had a fever of 101 and my husband ask the nurse what the protocol was for a fever and he said he didnt know. I am still waiting for an itemized statement that I have requested several times. Pharmacy bill seemed very high for one day when I was moved to a different place and the bill for 8 days was half. I would never recommend this place to anyone for any reason. Stay away. Also 20 minute wait to go the the bathroom and I went myself. I had a double hip replacement.

Not happy with the care my mother is currently getting!!! The nurse call button is useless since it takes a minimum of 45 minutes for someone to actually check on her. If she fell out of bed and hit her head nobody would know 🤷‍♀️ Her blood pressure was also 197/90 and nothing was done. This is VERY upsetting to know my mom is not getting cared for as she should. And the 2 twits at the front desk ????? RUDE !!!!!

Well, starting out 3 hrs post admission, they cannot find my wifes orders for pain meds, even though she hand carried them from St Josephs Hospital and personally handed them to staff upon arrival. The room is small cold and dumpy and she was promised a private room and put in a semi private. These people are unreal. I should have listened to the Docs at Apple Valley Med Center about having her transferred there. Oh well, will keep everyone updated.

This is the second time my mother has been here for short term rehab. Both times she has had to wait extended periods of time to use the restroom. The idea of having 4 beds using one bathroom is in my opinion ludicrous. Also every time she voiced concern about having to wait she was told that they were short staffed due to people calling in sick. The staff was great, just not enough for the number of patients. There was never any semblance of schedule which is what she is used to. At 88 years of age any change of routine is hard for her. I seriously doubt she would ever come back here if need arose in the future. This stay has left a very bad impression on her as well as the rest of the family.

Keep your stuff away from their laundry. You will never see it again. They put my dad in an inflatable pool thing at night to keep him in bed. I think they should try it.
